GTA V crash: ERR_GFX_D3D_INIT "Failed Initialization"
Fixable on your PCERR_GFX_D3D_INIT
The game couldn't start up its graphics. Most often an outdated or broken graphics driver, an unstable GPU overclock, or an overlay app getting in the way.
Why it happens
GTA V hands its graphics setup to DirectX and your GPU driver when it launches. If the driver is old or half-updated, a GPU overclock isn't fully stable, or an overlay (Steam, Discord, NVIDIA, RivaTuner) hooks in at the wrong moment, that setup fails and the game closes with this error. Rockstar's own support points to outdated drivers, DirectX, or Windows first.
How to fix it
- Restart your PC once. Rockstar's error message asks for this and it clears stuck graphics processes.
- Update your graphics driver from NVIDIA or AMD's site. If the error started right after a driver update, do a clean install of the previous version instead (use the 'clean install' option, or DDU in Safe Mode).
- Remove any GPU overclock. Use the Reset button in MSI Afterburner (or your tuning tool), or close the tool completely before launching.
- Turn off overlays: Steam, Discord, and the NVIDIA App / GeForce Experience in-game overlay. Close RivaTuner and screen recorders too.
- Run Windows Update and install anything pending, then restart.
- Reset your graphics settings: go to Documents\Rockstar Games, open the 'GTAV Enhanced' folder (or 'GTA V' if you play the Legacy version), and rename settings.xml to settings.old. If you can't find it, your Documents may be under OneDrive\Documents. The game makes a fresh one with safe defaults on next launch.
- Verify the game files in Steam, Epic, or the Rockstar Games Launcher.
Still broken?
If it only crashes after a few minutes of play rather than at launch, run a GPU stress test to rule out the card or its power cables. Still stuck? Book a session and we'll go through it together.

Despite the wording, this usually isn't your PC running out of RAM. It's mods pushing past the game's built-in memory limit, a broken graphics driver, or odd page file settings.
